Medical Social Services is an important link between patient, family, medical team and community.  The Social Services personnel assist with discharge planning, patient and family counseling, and serve as a liaison to community services.  The Social Worker is responsible for coordinating and implementing services post hospitalization, such as home health and hospice, durable medical equipment, and transfers to nursing home, rehab and LTACH.
