Medical Social Services
Home care social workers assist in evaluating social and emotional factors that affect a patient's health. Services include:

  • Helping the patient understand, accept and follow the physician's plan of care
  • Working with community resources to provide the best possible living environment
  • Assistance in locating alternative financing for medication and other medical needs

Certified Diabetic Educator
A specially-trained clinician, certified in diabetes education, will provide evaluation and individualized instruction for patients with diabetes. Services include:

  • Assessing the patient's condition
  • Educating individual on disease management
  • Setting individual goals
  • Making food and nutrition recommendations
  • Evaluating outcomes

Personal Care Services
A nursing assistant, under the supervision of a registered nurse, provides patients with personal care services to ensure a safe environment.Services include:

  • Assisting with bathing
  • Aiding in dressing
  • Assisting with exercise programs
  • Assisting with other activities of daily living

Telehealth
Telemedicine is technology using state-of-the-art equipment to monitor important patient information resulting in better patient care and reduced hospitalizations. This service is a supplement to skilled nursing visits. Equipment to monitor blood pressure, pulse, weight and oxygen saturation is placed in the patient's home. The information is then transmitted to our central station and is used by health care providers to make timely decisions resulting in improved patient care.
