Cardio Pulmonary Rehabilitation

CardioPulmonary Rehabilitation is appropriate for the following patients:

  • An individual who has had a heart attack, coronary artery bypass surgery, stents, valve replacement,or a cardiac transplant is a candidate for Cardiac Rehabilitation, which can be up to 36 sessions depending on the insurance policy.
  • Individuals with a diagnosis of chronic obstructive pulmonary disease, to include asthma, chronic bronchitis, and emphysema or the diagnosis of restrictive lung disease to include pulmonary fibrosis and sarcodosis, qualify for the Pulmonary Rehabilitation program if their breathing capacity is diminished 40 percent or more. This program is typically 24 sessions.

The multidisciplinary staff includes an exercise physiologist, a registered nurse, a registered respiratory therapist, a registered dietitian, a mental health counselor, and a smoking cessation coach. Diabetes education, provided through the Diabetes Care Center, is also available.

The CardioPulmonary Rehabilitation program is nationally-certified by AACVPR and is part of the LiveWell Center of Forrest General. Physician referral is required to participate in this program. Patients who complete this program are then eligible for Forrest General Wellness' Fitness Works exercise program.

Cardiovascular Disease Management Program

This program is designed for patients who do not qualify for Phase II CardioPulmonary Rehab, but need one-on-one monitoring prior to starting self-managed exercise program, and guides patients through an exercise and nationally-recognized education program to encourage an active lifestyle.

Exercise prescription and electrocardiogram (ECG) monitoring are a part of each exercise session. Patients who complete this program are then eligible for Forrest General Wellness' Fitness Works exercise program.
